Position Summary

Under supervision from the Senior Manager, Community Education, the Community Health Navigator (CHN) will promote, educate, and seek to improve the health of individuals by helping community members navigate and gain access to community services and other resources. The CHN will increase awareness on the importance of blood, organ and tissue donations from traditionally underserved populations.

Responsibilities

  • Plans and prepares marketing strategies with Sr. Manager, Community Outreach Education
  • Educates and builds awareness of Versiti’s mission of blood donation, tissue and organ procurement
  • Assists targeted community with basic social service needs, including: access to health and dental providers, emergency, housing, transportation, and mental health, safety
  • Maintains comprehensive electronic client files (including client notes, release of information, etc.), complete and submit monthly reports documenting activities, service plans, and outcomes achieved
  • Guides patients and provides service plans for effective management of chronic health conditions and self-care
  • Facilitates communication between client and provider as needed
  • Performs other duties as required or assigned which are reasonably within the scope of the duties in this job classification
  • Understands and performs in accordance with all applicable regulatory and compliance requirements
  • Complies with all standard operating policies and procedures

Qualifications

Education

  • High School Diploma High school diploma or equivalent required

Experience

  • 1-3 years Community Health Worker Training/Certification must be successfully completed within 24 months of employment required
  • 1-3 years Experience in community education, health, community outreach, social service, community advocacy or education required

Knowledge, Skills and Abilities

  • Effective interpersonal and communication skills and ability to communicate and collaborate with others in challenging and stressful situations required
  • Thorough grasp of patient’s needs and flexibility to work independently and with a team required
  • Active listening skills required
  • Effective organizational skills with the ability to prioritize and handle multiple tasks at one time while maintaining attention to detail required
  • Ability to read and interpret documents such as Standard Operating Procedures, safety rules, operating and maintenance instructions, and other department manuals. required required
  • Must have basic mathematical aptitude required
  • Ability to apply judgment to written or oral instructions required
  • Ability to learn/utilize computer applications such as Impact 211, Intranet site, knowledge of social media required

Licenses and Certifications

  • Valid driver's license required
